Please complete the following forms and bring them to your first session.

If you are an adult, please read and complete the Professional Disclosure Statement,

the New Client Information Form (Adult Version), and both HIPAA forms.

If you have Tricare insurance, please also complete the Tricare Medical Oversight Form.

If you are bringing your child or teen, please read and complete the Professional Disclosure Statement, the New Client Information Form (Child/Adolescent Version),

and both HIPAA forms.
